In September of 2020, the 1889 Jefferson Center for Population Health implemented the Community Care HUB for Cambria and Somerset Counties. The HUB model aims to impact health outcomes by addressing risk factors with the help of a community health worker. Jeannine McMillan is the Executive Director of the HUB and a member of the Healthcare Sector of the Cambria County Drug Coalition. She joins us with more about the program.
