Official Rules and Regulation for WATM ABC 23 / THIS 23.3 / WWCP FOX 8 and Envision Laser Centers LASIK Giveaway:
1. No purchase necessary. By entering this contest, you agree and understand that your information will be shared with the contest sponsor (Envision Laser Centers) but will not be sold or shared with any other agency. The contest will begin on Monday, April 15, 2013 and end on Sunday, June 16, 2013. (Contest Period).
2. One grand prize winner will randomly be selected on Monday, June 17, 2013, from all eligible registrations to receive Envision Laser Centers' Lifetime Custom LASIK eye surgery. Lifetime Custom LASIK eye surgery includes all pre and post-operative eye exams for 1 year and any enhancements (additional fine tuning surgery) for lifetime. Also includes surgery performed by Ketan H. Patel, MD, the area's most experienced LASIK surgeon. We offer the newest LASIK technology like "ACE" eye tracking, iris registration that allows the laser to recognize each patient and deliver the most precise treatment possible. Envision Laser Centers will also include our comfort plugs that keep moister in the eye to promote faster healing. (Retail Value - $3,948.) In order to be eligible, registrants must submit entry online at www.abc23.com or www.fox8tv.com. Registrants must meet specifications as outlined in items 3-23 of the official rules and regulations.
3. 100 second place winners will be selected to receive a $1,000 gift certificate good towards our Lifetime Custom LASIK @ Envision Laser Centers. No cash value and cannot be applied towards any other procedure or assurance package. Certificate must be presented at time of consultation. Registrants must meet specifications as outlined in items 3-23 of the official rules and regulations.
4. Entries must be received by midnight on 6/16/13 in order to be eligible.

7. Registrants must be at least 18 years of age and winner must go through a complete eye exam to be deemed a good candidate. If winner is not deemed a good LASIK candidate, another name will be drawn from list of entries.
8. Grand Prize Winner Patient Lifetime Enhancement requirements: Patients must attend all scheduled post-operative appointments in order to maintain assurance policies. If an appointment is cancelled and the time frame for the appointment has lapsed, enhancement packages are voided. Enhancement packages do not include eye care with unrelated symptoms to LASIK, ie. Pink eye. Annual dilated eye exams are required every year to maintain the Lifetime Assurance Plan and are NOT covered under the initial LASIK fee. The exams may be done by any LASIK experienced optometrist.
9. Custom LASIK procedure will be performed at either our Altoona or Johnstown surgical facility.
10. Prize is non-transferrable and expires 1 year after contest on June 17, 2014.
11. The odds of winning will depend on the number of eligible entries received.
12. The prize drawing will take place on Monday, June 17, 2013, and winner will be notified at the email listed on the registration.
13. Grand Prize must be collected in person at WWCP FOX 8/WATM ABC 23/THIStv 23.3, 1450 Scalp Avenue, Johnstown, PA 15904, no later than 7/17/13.
14. Prize is non-transferrable. No substitutions or exchanges will be allowed. No cash value. Winning contestant is responsible for the expense of taxes and any transfer fees.

23. In the event that the grand prize winner does not meet the requirements as outlined above, an alternate winner will be selected from the remaining qualified contestants. Any alternate winner must meet the contest requirements as set forth. This alternate selection process shall continue until a qualified winner is selected.
